Start enrolling a second factor

POST
/security/mfa/setup
Returns the secret and the otpauth:// uri an authenticator scans,
once. Nothing is guarded until a code proves the app actually has it.
Starting again before confirming replaces the unconfirmed secret.
Under enforcement E3 an admin without a confirmed second factor is
answered 403 mfa_required on every admin route, pointing here; this
route and /security/mfa/confirm are the account's own, never
admin-only, so an admin is never locked out of them.
